Missing Journal/Article Request Form

The following form is intended to be used if you are unable to locate a journal (bound or individual) or pages are missing from an issue, please fill out the following information and return it to the Reference Desk or the Serials Department in the library or click on the SUBMIT button at the bottom of this page. The Serials Department will conduct search proceedings within 72 hours of receiving this form.
