| Individual Research Appointment Request | An individual appointment provides an opportunity for a student to meet individually with the Reference Librarian. Research assistance will include a discussion of library and web resources as well as research strategies. Contact: Michael Smith |
| InterLibrary Loan Request - Books | This form is intended to be used by Elms College's students, faculty, and staff to request books and various media that are not owned by Alumnae Library. Contact: Holly Stevens or Michael Smith |

| Missing Journal or Article Request | Patrons searching for specific print journal issues, in the library, but are unable to locate them, may ask the library to search for the item using the missing journal or article form. If patrons locate the print journal issue, but find the article is missing, they may request the article through this form. Contact: Debra Gomes |
| Book Memorial Request | Leave a lasting memorial for a loved-one. Honor an employee, graduate, or volunteer. Commemorate a birthday, retirement, or anniversary. Contact the library to select a memorial book or fill out this online form. Contact: Anthony Fonseca |
| Information Literacy Session Request | This form is for use by instructors who wish to schedule a library instruction session for a class. Contact: Michael Smith |
| Library Materials Request | Faculty members may request an order for library materials from departmental allocations of library money. Requests may be made in person or using this online form. Contact: Anthony Fonseca or Lynn Gamble |
