3. Apply for Financial Aid
If you haven’t done so, fill out the Free Application for Federal Student Aid (FAFSA). The Elms College code is: 002140

4. Health Forms
You will need to submit the following forms to the Elms College Health Center
Graduate students in Business, Education, MSW, & (IN-PERSON BioMed, BioTech, & PostBac) are required to submit the following information:
Required Medical Forms
- Meningitis Form (PDF)
- Report of Medical History Form (PDF)
- Physical submission (PDF)
- Immunization Form (PDF)
- Tuberculosis Screening Form (PDF)
You may be able to obtain your vaccination records using the Massachusetts Immunization Information System at: https://myvaxrecords.mass.gov/
MSN & DNP students are required to submit the following information:
- Report of Medical History Form
- Tdap within 10 years
- MMR, series of 2 vaccines or positive lab titers for each disease
- Varicella, series of 2 vaccines or a positive lab titer
- Hepatitis B, series of 3 vaccines or a positive titer
- Meningitis vaccine within 5 years or a signed and dated Meningitis Waiver Form
*The above information is required for admission. In addition, Clinical Clearance documentation will be collected before the start of practicum by your program coordinator.
Graduate students in ABA, Data Analytics & (ONLINE BioMed, BioTech, & PostBac) are required to submit the following information:

9. SASS
Student Accommodations and Support Services (SASS) collaborates with and empowers students who have diagnosed disabilities in order to coordinate support services and programs that enable equal access to an education and college life.
SASS works to foster academic success, personal responsibility, self advocacy, and leadership while addressing the unique needs and interests of individuals who have registered with the office. SASS ensures that Elms College operates in accordance with federal regulations as outlined in the Americans with Disabilities Act, Section 504, and other pertinent regulations.
Incoming students may be required to complete a placement test for English and/or foreign language courses. Students with disabilities are entitled to receive reasonable accommodations for these placement tests.
